What missed appointments and slow replies cost your clinic, and how AI helps

Empty slots and unanswered enquiries are the two quietest ways a clinic loses money. Here is how to put a number on both, what the evidence says about reminders and reply speed, and where AI fits.

Most clinics know their no-show rate roughly. Far fewer know what it costs, or how much revenue slips away while enquiries sit unanswered over a weekend. Both are fixable, and both are places where well set up AI can take work off reception without making the clinic feel automated.

GP and practice appointments were missed each year in England, about one in twenty, at an estimated cost of more than £216 million, based on around £30 per appointment.

How much do no-shows cost a private clinic?

NHS figures are useful context, but a private clinic feels a missed appointment directly: the practitioner is paid, the room is booked and the slot cannot be resold at short notice. Working out your own number takes five minutes.

Do reminders really reduce missed appointments?

the attendance rate with text message reminders compared with no reminder, across seven studies and 5,841 people. Text reminders worked about as well as phone calls, at a lower cost per attended appointment.

Reminders do not need AI. Your booking system should already send them. Where AI helps is everything around the reminder: writing preparation instructions that fit the specific appointment, replying when a patient asks to move, offering the freed slot to someone on the waiting list, and doing all of that within minutes rather than the next morning.

What does a slow reply to an enquiry cost?

~7×

more likely to reach a meaningful conversation when companies replied to an online enquiry within an hour, compared with waiting even one hour longer. The study audited the web enquiry response times of over 2,000 US companies.

That research is about businesses, not clinics, but the pattern is familiar to anyone who runs reception. A patient comparing three clinics at 9pm usually books with whoever answers first and answers well. Enquiries that arrive at evenings and weekends are often the ones that go cold.

Where does AI fit at reception?

  • Draft replies to common enquiries using your real prices, availability and treatment information
  • Flag anything that mentions symptoms, medication or a complaint for a person, instead of replying
  • Write appointment specific preparation and aftercare messages for staff to send
  • Handle reschedule requests quickly and offer freed slots to the waiting list
  • Summarise the overnight inbox so the morning starts with a clear list

What should stay with a person?

Anything clinical, anything sensitive and anything where the patient is worried. AI drafts and sorts; reception decides and sends. That keeps the personal touch that private patients pay for, while taking the repetitive part of the job away.

How do you measure the difference?

  • Track your no-show rate monthly, by appointment type
  • Measure the median time to first reply for web and email enquiries
  • Count how many freed slots are rebooked
  • Set targets from your own baseline, then review after six weeks

Frequently asked questions

NHS England estimated in 2019 that around 15 million GP and practice appointments were missed each year, costing over £216 million at about £30 each. For a private clinic, multiply your monthly missed appointments by your average appointment value to get your own figure.

Yes. A Cochrane review found text reminders increased attendance compared with no reminder, and worked about as well as phone calls at a lower cost per attended appointment.

It can draft replies to routine questions, but in a clinic a member of staff should approve what goes out, and anything mentioning symptoms, medication or a complaint should go straight to a person.

As fast as you can do it well. Research on online enquiries found replies within an hour were far more likely to lead to a real conversation than slower ones. Measure your current median reply time and set a target from there.
