A no-show at a blood testing clinic is quietly expensive. The phlebotomist is paid, the room is ready, and the slot cannot be resold once the time has passed. Early morning slots hurt most, because fasting patients need them and there are only so many. The good news is that most no-shows are predictable, and most of the fixes sit in how appointments are booked and confirmed.
of outpatients missed their appointment when the text reminder stated what a missed appointment costs, against 11.1% with the standard reminder. Changing a few words cut missed appointments by about a quarter.
Source: Hallsworth et al., PLOS ONE, 2015Why patients miss blood test appointments
- They forgot. The booking was weeks ago and nothing reminded them
- They could not make it, and not turning up felt easier than phoning to cancel
- They ate breakfast before a fasting test and assumed there was no point coming
- They never quite committed: nothing was paid, so nothing was lost
Send reminders that do some work
One reminder the evening before beats several vague ones. Make it specific: the time, the place, and how to prepare, including the fasting step if there is one. The research above shows the wording matters too. A plain line about what a missed slot means, such as another patient waiting for it, is more effective than a generic "see you tomorrow".
Keep test names out of text messages. Reminders appear on lock screens and shared phones, and "your appointment" does the job without revealing anything about someone's health.
Make cancelling easier than not turning up
If changing a booking means a phone call during opening hours, many people will simply not come. Put a link to change or cancel in every confirmation and reminder. Let it work until a set point, say 24 hours before, and put the freed slot straight back online so someone else can take it.
Ask for payment when they book
A booking that is already paid for is a commitment, not a maybe. Taking payment online at booking, or at least a deposit, gives patients a reason to turn up or to cancel properly. Be clear about the refund rules at the moment they pay, so nobody feels caught out.
Track no-shows so you know what is working
You cannot improve a number you never see. Mark each visit as completed or a no-show on the day, and watch the weekly figure. Then change one thing at a time, reminder wording, timing, or payment, and see what moves.
(opens full size in a new tab)A quick checklist
- One clear reminder the evening before, with time, place, and preparation
- No test names in texts
- A change or cancel link in every message
- Freed slots go straight back online
- Payment or a deposit taken at booking
- Every visit marked completed or no-show, and the weekly rate reviewed
The day plan above is from Drawn, the booking system for blood testing clinics we are building, which handles reminders, change links, and no-show tracking. There is a short video of it below.
